What is a medical home?

A medical home has many services located in one facility. This provides access to services that would require you to go to many different places. We have clinical, pharmacy, radiology, laboratory, social service nutritional and educational all under "one roof".

Our staff coordinates care with all your providers assuring there is no duplication of services. Medication lists are continually updated to make sure the patient understands what and why they are taking each medication. Care is not necessarily centered on the caregiver visit but rather care is delivered by members of our team to the patient at the patient's convenience.

How do I make an appointment?

Follow-up appointments can be made at the end of each visit or by calling 404-231-4431.

What if I have to cancel my appointment?

Please allow 24 hours notice when cancelling appointments. This will allow other individuals who may not be feeling well easier access to appointments. There will be a $25 charge for appointments that are not cancelled 24 hours in advance.
